Enrollment

· Complete a general application (download here) return to the church office.

· PreK3 & PreK4 Registration fee of $140.00 (Non-refundable) must accompany the application. (Includes Bag)

· PreK2 Registration fee of $105.00 (Non-refundable) for the 3 day program must accompany the application. (Includes Bag)

· Students must be 2, 3 or 4 years old by September 1 of the school year which they are applying.

· Need a copy of their Birth Certificate.

· Need a current copy of the Ga. Immunization Record #3231.

· PreK3 & PreK4 must be potty trained as we do not allow pullups.

STUDENTS CANNOT ATTEND THE PROGRAM UNTIL WE HAVE A CURRENT SHOT RECORD ON FILE.
